In order for your child to receive this vaccination when we visit your school on WEDNESDAY 8^TH OCTOBER 2025, please complete the e-consent form by THURSDAY 2^ND OCTOBER 2025

• Please visit www.surreyimmunisations.co.uk and enter your school’s unique code SR143001 (For Years R – 2)

• If you do not want your child to receive a vaccine, we would ask you to still complete a consent form but select the ‘No’ option.

• If you do not want your child to receive the nasal spray vaccine on the basis of its porcine gelatine content, please fill in the ‘No’ box indicating this, and you will be given the opportunity to consent for an alternative vaccination. This vaccination is delivered by injection into the arm and does not contain any porcine gelatine.
